Methinks your leveling progression needs to be updated.. and also your topic. you say 5 fighter in the progression and take levels 6-39 twice.. either that or i can't read

Not really that important but skills are:
Concentration: 43 Discipline: 43 Spellcraft: 42 Tumble: 42
I would change your character name. An Iconic druid would be conjuration based, not evocation. Also an Iconic druid would have animal empathy as a skill. I question the level of fighter, which will add weapon proficiancies, as opposed to a second level of monk for evasion.
Otherwise, its a nice build. The name just annoys me. But that's really not that important.
Monks get Evasion at level 1, the addition of fighter allows the use of heavy armor if the heavy armor has better stats for the shifted shape. That and a bonus feat.

I kinda agree on the name I couldn't think of anything and having 95% druid levels somehow qualifies as Iconic!
I actually took the fighter level to get to use the longbow and for the extra feat. Longbow is really the only way to make any kind of damage when not shifted.
