The concept of this build: Use IKD with your two-bladed sword, which counts as a large weapon. Once they're on the ground, get a whole lot of sneak attacks, because your two-bladed sword counts as dual wielding. Make sure they never get back up with Death Attack. Keep getting ridiculous numbers of sneak attacks. If that damage doesn't kill them immediately, wait for them to fail their Devestating Critical save (DC 39). All in all, I've found the build to be extremely effective.
Skills: Get Hide and Move Silently to 8. Max Discipline on Fighter levels. As Rogue and Assassin, max Tumble, UMD, Open Lock, Search, and Disable Trap. Max Spot and Listen until they hit 33. Don't take Tumble past 40. Don't cross-class, ever, on this build. That will take up all your skill points.
Comments? Questions? Corrections? I tested it out (in combat this time, so it really works, unlike my AA variant), so no worries there.

Its an interesting build, getting you where you want. A few things; You have EWS, smart vs those opponents who are immune to crits, to increase dmg output.
A char with high discipline f.ex a warrior cleric will be hard to knock down, while he smacks at you. They can knock you back, you know. So here is the main thing I would try to improve. Why so low Hide and MS? Getting in the first sneak/death attack is important, it can be a huge difference in this build. Or are you trusting invis to take care of that?
What armor are you planning to use? Heavy? More stats to, please, final AB, and ac with chosen armor.Your death save will be low to. 22 without int buffs I think.
Improvement tips, after quick test of your build right now. 1. Start out with STR 16, INT 14, CON 14. 2. At lvl 27, take assassin and Great Strength feat 3.At lvl 36, take the 5th fighter lev to get EWS, here you will lose imp.sneak attack.
edit: It will get you a higher discipline to, the fighter lev coming later
Overall you lose only 1d6 sneak damage, but the gain is 40hp, 43 more skill points and your save DC vs death attack improve by 1.
All in all a good build. Fill up your stats for us now

I would agree with Grimnir on most of that. What would concern me is that AC at lower levels. You may want to take 1 level of fighter at Level 2 to get those Armor Proficiencies.
Also, I would suggest starting your Strength at 16 and move one of those points into Int, and the other two into Dex to give you an even 16 Dex. You can then use that Great Strength feat for Epic Prowess to make up for the 1 loss in AB.
In addition, since he is Strength-based and since he also has EWS, I would advise dropping 1 or 2 of those Improved Sneaks to help balance the character. I would suggest Armor Skin as well as either Epic Will(this guy's Will save is abyssmal) or Slippery Mind, or both.
Finally, perhaps it's just a bug I'm unaware of, but I thought Great Cleave had to be taken before Overwhelming Crit, yes? _________________ Got Hommlet? World of Greyhawk Action Server (with 1/2 price ales on Mondays!)

Right on all counts. I realized I'd been wrong about IKD this morning. The Great Cleave was a sloppy edit during testing, sorry about that. Simple fix, just switch the order on that and Overwhelming Crit. Before I read the suggestions above, I'd already remade the build for kukris and a little more strength. Here it is:

Here it is. I wasn't able to work in Epic Will, so the build is still pretty pathetic on will saves. I hope Slippery Mind will help some with that. Here's the build:
