Using a rapier, depending upon your PW you may want to use shortsword, handaxe (opens up Keen Edge) or q-staff (2-handed) Damage: 1d6 +12 (18-20/x2) +7d6 Sneak Attack Damage (buffed): 1d6 +18 (18-20/x2) +7d6 SA (1d6+17 reciprocal)
There is scope for some flexibility, you may want to drop the intitiative feats to open up Martial Weapons, and another Epic Feat. The skill set is also flexible, and Taunt could be included, I just find it a pain to get the Taunts 'off' during combat. Another option is to drop Dex some more, and take the Heavy armor feat. With Cat's Grace you could drop Dex to 10, and increase Str or Int to 16 depending on your preference.
I didn't find a build that had more than 5 levels of Bard, in this combination - that may be becuase it is a useless class distribution. .

First of all, nice layout.
Second, I think it looks good. I would be tempted to go Bard12/RDD4/Rogue4, if only to open up 4th level spells faster and to get your song to mature as well.
I would also advise dropping SF: Spellcraft for Extend Spell. And then move that up on the feat pecking order.
Since the bulk of your spells will be precombat, you could drop Concentration in lieu of Taunt, but like you, I have mixed success with that skill.

That's good. I'd probably drop Rogue to 10 levels and put Bard at 20 for LI.
