This build is meant to fight unarmed to use the Monk bonuses to AC along with some dex bonuses. If you prefer using armor then you can increase CON by 2 and lower wisdom by 4 but I think it works better this way.
Feel free to take him apart:D I see 2 main problems -
1 - if you go with unarmed, you should really take 6 more monk levels to get the max unarmed damage. So I would take Kama over unarmed with this level split, which also frees up the glove slot.
2 - Low AB. I would drop ESF: Disc and Epic Reflexes for more Gr. Str. I'd also drop 4 monk levels for 4 Fighter levels (end at Monk 6), go with kamas, and move EWF adn EWS to fighter bonus feats to get 2 more Gr Str. Gains 2 more AB to reach 40, and 2 damage.

Go kama and take more STR, like Tatooed Monk said, I don't understand why you lists heavy flail, monks fight best with kamas, it's just natural to use them, with kamas your attack spread will change to 38/35/32/29/26/23 and you can use Flurry of Blows for another APR at -2 AB, Heavy Flail is just no good.

Taking 6 Monk instead of 10 like TM suggested will actually net another BAB for +3 AB and +2 damage.

Also, I'd drop initial WIS to 8 to boost CON to 18 and INT to 12, I think you'll need to take Fighter at level 1 to drop WIS under 11 but that's no problem in any way. WIS only gives you 2 AC, but heck you're a Dwarven Defender, you don't need AC, you've got DR 27/-, you need STR. Starting with more CON means you'll spend less points for EDR and you'll end with 2 points higher STR, +1 AB and damage.

I'd even drop Armor Skin for Great STR and leave CON at 21, that will net another 2 STR points.

Another choice would be to drop WIS to 8, DEX to 13, leave INT as it is, and boost CON to 19, that way you won't need to lose either Armor Skin or CON 22, only one of them, though you lose 1 DEX AC, it nets higher with Armor Skin, or HP instead, which I find more valuable than AC.

When using a Dwarven Defender your AC is somewhat useless, as regular melee won't hurt you, HP would be more important than AC for when you fight mages...

I took heavy flail because I wanted a blunt weapon and I wanted a twohander since the monk cant use shields anyway. But I guess damage output will be better with the kama in most cases.
I quickly levelled a character to monk4/fighter10/DD26 with starting stats

took 5 great str feats and both epic prowess and armor skin for a final ac of 21 and ab of 42/...
Slightly weaker saves for this version and not as fast as the other version, but perhaps more effective. No IDK this way however. I guess I could make it monk6/fighter8/dd26 instead and drop one epic feat. Fighter gives you automatically Shield Proficiency, so you can use them, not that it is of any practical use, Dwarven Defenders don't need AC. What I meant was that if I use a shield I lose the monk bonuses so I would get the same ac without a shield as with a shield and therefore I might aswell use a twohander. But ofcourse it may be better to use a kama instead. I looks cooler with a twohander to me though Note that if you use a shield, you also lose the monk attack progression with a kama.

If you plan to use a shield or a non-kama weapon, I would drop monk completely and take Rogue instead. You gain some sneak damage, keep the tumble dumps, and add other skills like UMD, set/disable traps, etc. Ro 10/F 4/DD 26 would work well, although you could take as few as 2 Rogue levels for more Fighter bonus feats.

TM Monk does give you Cleave and IKD, so it's not completely worthless to take monk, but taking monk without using its APR progression + Flurry doesn't fit in my brain.
