Well, there was a little "contest" to get the max AB build some time ago, I think these were the ones:
Melee:
Highest Possible STR based effective AB (Druid 1/Fighter 11/WM 28) by Mithdradates
Highest POSSIBLE MELEE AB (Fighter8/Pal4/WM28) by Grimnir77
These builds actually suck as they are able to hit and that's it, they would die really fast.
Ranged:
The REAL highest POSSIBLE AB (Bard 16 / AA 23 / Druid 1) by Nimueh_Leafbow
